Each selection is one complete finished-treat pack: 20 × 4 oz bags (5 lb), 10 × 8 oz bags (5 lb), 7 × 12 oz bags (5.25 lb), or 5 × 1 lb bags (5 lb). Choose as many packs as you need for each treat. The $150 white-label order minimum applies to the overall order before shipping and tax.

What to enter

List each flavor on a separate line in the flavor and quantity field. Example: Pumpkin Patch Crunch Bites — crunchy — 4 oz — 2 complete cases (40 finished bags / 10 lb). If you want more than one bag size or color, list each combination separately in that field and explain it in Additional Details. The single preferred-size and bag-color selectors indicate your main choice; your written breakdown controls what we review for the quote.

Full-Service: choose a front and a back label finish separately. Ready-to-Label: select “Not applicable — customer-supplied labels” for both finish fields; you provide and apply the finished labels, and we fill and seal the bags. Do not enter or upload confidential artwork or barcode files here. If requesting UPC placement, provide the valid customer-owned code securely during the later proof process.

No payment is taken through this form. The written flavor breakdown is reviewed manually; the form does not automatically calculate pack totals or enforce the combined $150 minimum.

Do not submit sensitive artwork here; customer files are collected privately after an order is arranged.
